Caregiving Specialist

We are seeking a skilled and compassionate Registered Nurse to join our team as a Weekend Home Health RN.

Your Key Responsibilities:

• Conduct thorough assessments and develop personalized care plans to meet patients' unique needs.
• Provide education and support to patients and their families on medication management, therapy, diet, and exercise.
• Collaborate with healthcare professionals to ensure seamless transitions and coordinate discharge from services.

To be successful in this role, you must have:

• A valid RN license in Kansas
• CPR certification
• Knowledge of state and federal home health regulations
• Clinical proficiency in nursing specialty and strong communication skills
