**Salary:**

$150,000 - $200,000 per year based on experience and location.

**Job Description:**

We are seeking an Advanced Hospice Care Specialist to join our team. As a hospice nurse practitioner, you will work closely with patients and their families to alleviate burdensome symptoms and align medical treatments with their goals and values. You will also collaborate with a team of clinicians and teammates to deliver high-quality care in patients' homes.

**Required Skills and Qualifications:**

To be successful in this role, you will need:

  • a graduate degree in nursing from an accredited institution;
  • at least 2 years of experience in chronic disease management or long-term care; and
  • a strong passion for providing quality care to patients with advanced illnesses.
