Job DescriptionThe Program Manager will be responsible for leading a team of experienced Direct Support Professionals who provide 24/7 support to individuals with developmental disabilities in their own homes. Key responsibilities include:
- Fostering open communication between clients, families, and community organizations to ensure seamless coordination of services.
- Coordinating meetings related to client care plans, including life plan reviews and residential plan updates.
- Implementing and following up on recommendations from multidisciplinary teams to ensure optimal client outcomes.
- Serving as liaison for involvement in support groups, self-advocacy initiatives, vocational programming, transportation services, and integrated community activities.
- Collaborating with medical professionals to review medical documentation and ensure timely follow-up on health issues.
The ideal candidate will possess exceptional leadership skills, with a focus on critical thinking, clear communication, and follow-through. Strong organizational and problem-solving abilities, along with sensitivity to the needs and expectations of individuals with developmental disabilities, are essential.
Required Skills and QualificationsTo succeed in this role, candidates must have:
- A bachelor's degree or equivalent experience in a field related to disability support or social work.
- A minimum of three years of experience working with individuals with developmental disabilities, preferably in a supervisory capacity.
- Proven ability to address multiple demands simultaneously, prioritize tasks, and maintain a high level of organization.
- Excellent listening and comprehension skills, with a strong emphasis on effective communication and interpersonal relationships.
Job Path offers a competitive salary of $49,100 per year, with opportunities for growth and professional development.
