Key Responsibilities

  • Conducting assessments, Minimum Data Set (MDS), and care plans for designated residents
  • Overseeing the timely completion of MDSs by various departments
  • Ensuring precise and prompt completion of the MDS/RAPs/Triggers documentation for assigned residents
  • Initiating care plans aimed at achieving optimal resident outcomes
  • Creating and distributing monthly care plan schedules
  • Facilitating care plan meetings
  • Maintaining and revising care plans and assessments for residents
  • Regularly reviewing resident incidents and accidents
  • Monitoring the completion of 7-day look-back documentation by nursing personnel
  • Conducting audits of clinical record documentation
  • Engaging in continuous education to remain compliant with regulations
  • Executing all job responsibilities safely and reporting any unsafe conditions
  • Upholding superior customer service standards
  • Adhering to facility policies and procedures

Qualifications

Education: Graduate of an accredited nursing program

Experience: Minimum of one year in a long-term care setting with familiarity in MDS and Care Plan processes, as well as basic knowledge of PPS and RUGs

Additional Requirements:

  • Must hold a Registered Nurse license
  • Ability to perform heavy lifting, stooping, bending, and reaching
  • Capability to push and pull heavy carts
