Job Summary

We are seeking an experienced Optometrist to join our team as an Independent Eye Care Professional. As an Independent Optometrist, you will have the autonomy to establish your own practice within our Sam's Club locations, providing high-quality eye care services to our members.

Key Responsibilities

  • Provide comprehensive eye exams and vision care services to patients
  • Establish your own schedule and fee structure
  • Have access to equipment and instrumentation for a fair market value
  • Option to opt into administrative and/or Opt-Tech support services (if state allows)

Requirements

  • Doctor of Optometry (OD) from an accredited optometry school
  • Valid state optometry license
  • General Liability Certificate
  • Professional Liability Insurance Certificate
  • W-9
