About The Position

The Home for Little Wanderers is seeking a skilled and compassionate Youth Transition Coordinator to join our residential program. In this role, you will work closely with youth and their families to develop and implement transition plans, ensuring a smooth and successful transition to independence.

Responsibilities:

  • Collaborate with youth and their families to identify and prioritize transition goals.
  • Develop and implement individualized transition plans, incorporating education, employment, and life skills training.
  • Connect youth with community resources and services to support their transition to independence.
  • Maintain accurate and timely records of client progress and transition plans.

Qualifications

To be successful in this role, you will need:

  • A Bachelor's degree in Psychology, Sociology, Social Work, or Education, or a closely related field.
  • One year of experience working with youth in a residential setting.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse groups.
  • Computer proficiency, including Microsoft Office and Internet Explorer.
