Job SummaryWe're seeking a highly skilled and compassionate Primary Care Physician to join our team at ChenMed. As a Primary Care Physician, you will be responsible for providing high-quality, patient-centered care to our seniors, and working collaboratively with our care teams to achieve the best possible outcomes.
Key Responsibilities- Provide comprehensive primary care to patients with acute and chronic illnesses, including assessment, diagnosis, treatment, and management of their health needs.
- Develop and implement individualized care plans that take into account the unique needs and goals of each patient.
- Work collaboratively with our care teams, including care promoters, care facilitators, and care coordinators, to ensure seamless care coordination and communication.
- Participate in quality improvement initiatives and contribute to the development of best practices in primary care.
- Stay up-to-date with the latest evidence-based medicine and clinical guidelines to ensure that patients receive the highest quality care.
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty required.
- Board certification in Internal Medicine, Family Medicine, Geriatrics, or similar specialty preferred.
- Current State Medical License or ability to obtain one.
- Basic Life Support (BLS) certification from the American Heart Association (AHA) or American Red Cross (ARC) required.
