About the Role

We are seeking a skilled and compassionate Registered Nurse (RN) to join our team as a Home Health RN. As a Home Health RN, you will provide high-quality patient care in the comfort of their own homes, working closely with a multidisciplinary team to develop and implement individualized care plans.

Key Responsibilities
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing targeted interventions.
  • Develop and implement individualized care plans, working collaboratively with patients, families, and healthcare teams to ensure seamless care transitions.
  • Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians.
  • Monitor patients' progress, adjusting care plans as needed to ensure optimal outcomes.
  • Communicate effectively with patients, families, and healthcare teams, ensuring that all parties are informed and engaged in the care process.
Requirements
  • Active Registered Nurse (RN) License in Colorado.
  • Graduate of an accredited school or program of nursing.
  • At least 1200 hours of experience as an RN.
  • Healthcare CPR/BLS Certification (demonstration course required).
  • Home health care experience preferred.
  • Pass a background check and drug test (we are required to test for THC).
