MDS Nursing Coordinator

POSITION SUMMARY:
The MDS Coordinator is a seasoned healthcare professional responsible for ensuring precise assessment coding and maintaining current care plans for all residents, in collaboration with or as the RN Coordinator. This role is crucial in establishing reimbursement rates for eligible residents through accurate assessment coding and contributes to quality metrics utilized for public reporting. The MDS Coordinator guarantees that documentation related to resident care aligns with the care plan and accurately reflects the services provided.

ESSENTIAL DUTIES:

1. Must possess the physical and mental capacity to perform routine job responsibilities.

2. Should have a comprehensive understanding of nursing services, nursing administration, rehabilitation, general nursing, geriatric nursing, the MDS process, and its electronic medical record (EMR) applications.

3. Requires strong managerial, organizational, planning, computer, and interpersonal skills.

4. Capable of initiating, completing, and updating care plans with each MDS assessment in accordance with applicable practice scope.

5. Ability to conduct training sessions for nursing staff on care plans, documentation, activities of daily living (ADLs), and EMR applications.

6. Proficient in monitoring resident EMR records for consistency and accuracy.

7. Skilled in identifying issues and effectively communicating them to the Director of Nursing, Social Worker, Activity Director, Diet Tech/RD, or other relevant facility personnel.

8. Must maintain a personable and professional demeanor with residents, families, and staff.

9. Should exhibit compassion, tolerance, and understanding towards the elderly population.

10. Knowledgeable about IDH standards and documentation methods in compliance with those standards.

11. Must be familiar with the Resident Assessment Instrument (RAI) Manual and navigate it effectively.

12. Understanding of the technical and clinical criteria necessary for Medicare beneficiary coverage.

KEY RESPONSIBILITIES:

A. Uphold the facility's care philosophy and work towards achieving its objectives.

B. Oversee the completion of the MDS during:

a. initial admissions

b. readmissions

c. quarterly assessments

d. annual evaluations

e. significant changes

f. and as required for PPS/Insurance reimbursement

C. Provide a monthly calendar of scheduled MDS assessments, ensuring the IDT can work from the In-Progress list in the EMR.

D. Ensure timely completion of each MDS section by the appropriate department, including staff signatures on the EMR.

E. Collaborate with departments to address any issues within their MDS sections and arrange for necessary re-education.

F. Review ICD 10 diagnosis coding and sequencing upon admission, reentry, and with each MDS assessment, completing Physician Queries for clarification as needed.

G. Conduct daily reviews of clinical records to identify opportunities for capturing the highest level of care on OBRA and PPS assessments, aligning reimbursement with care and resident characteristics.

H. Facilitate daily MDS-therapy meetings and weekly Utilization Review meetings.

I. Participate in pre-billing reviews of claims per facility guidelines.

J. Monitor MDS-based Quality Measures for coding errors and assist the IDT in identifying measures for facility QAPI efforts.

K. Support staff across departments in EMR charting, documentation, and achieving care plan goals in line with the resident's MDS.

L. Communicate resident care plans and progress to staff, residents, and families, ensuring timely care plan review dates are set in PCC with MDS assessments.

M. Conduct in-service training for nursing staff every three months on care plans and documentation.

N. Document, report, and follow up on discrepancies in documentation that do not meet program standards.

O. Collaborate closely with the Director of Nursing and other department heads to ensure consistency in resident care.

P. Work alongside the RN to coordinate resident care plans with therapy departments to ensure appropriate treatment when clinically indicated.

Q. Maintain confidentiality of resident information and respect their personal and property rights.

R. Adhere to established fire, disaster, safety, infection control, and evacuation policies and procedures.

S. Perform additional duties as assigned by the Administrator.

T. Assist with staffing coverage as necessary to meet resident needs.

QUALIFICATIONS:

1. Current State of Indiana license as a Registered Nurse or Licensed Practical Nurse.

2. Must be able to read, write, and communicate effectively in English.

3. AANAC certification is preferred.

PHYSICAL AND SENSORY REQUIREMENTS:
  • Ability to move intermittently throughout the workday.
  • Effective communication skills in English.
  • Ability to manage the mental and emotional demands of the position.
  • Must possess adequate sight/hearing or use prosthetics to meet job requirements.
  • Ability to work independently and collaboratively with residents and staff.
  • Must meet general health requirements as per facility policies.
  • Ability to relate to and work with ill, disabled, elderly, and emotionally distressed individuals.
  • Physical capability to lift a minimum of 25 pounds and assist in resident evacuation during emergencies.
WORKING CONDITIONS:
  • Work involves office areas and various locations throughout the facility.
  • Frequent movement during working hours.
  • Subject to interruptions and interactions with residents, staff, and visitors.
  • Exposure to varying temperatures in kitchen/storage areas.
  • May encounter hazardous conditions and infectious materials.
