NP or PA Complex Care Specialist

Optum is seeking a skilled NP or PA to deliver high-quality home-based medical care to complex, chronic patients in New Haven, CT. As a clinician-led care organization, we're creating a seamless health journey for patients across the care continuum.
Our Home-based Medical Care team is dedicated to bringing home-based medical care to patients who need it most. This life-changing work helps give older adults more days at home, where they can live comfortably and with dignity.
As a member of our team, you'll work collaboratively with our interdisciplinary team to develop and implement patient care plans that address the physical, mental, and social needs of our patients. You'll be responsible for performing initial comprehensive assessments, developing patient management care plans, and providing ongoing care to patients in their homes.
Key Responsibilities:
- Conduct initial comprehensive assessments on newly enrolled patients and provide ongoing care thereafter
- Develop patient management care plans and update them as needed
- Meet with patients and/or their legal representatives to review care plans and involve PCPs or supervising physicians as needed
- Perform basic procedures of nursing care, including IV placement, blood draws, injections, foley catheters, nasal packing, wound care, etc.
- Prescribe diagnostics and interventions to avoid unnecessary acute admissions
- Complete follow-up and post-discharge assessments according to standard operating procedure
- Consult with hospital, emergency, or post-acute clinical teams following patient transfers
- Educate patients and/or their legal representatives on disease processes and ways to manage them effectively
- Participate in ongoing meetings on patient care and clinical excellence
- Implement HEDIS measure campaigns and quality initiatives to ensure high standards of care
- Be on-call for caseload during business hours and participate in clinical after-hours on-call program on a rotational basis
- Stay current on medical and nursing research, technology, and related issues through continuing education courses, professional meetings, and journal reviews

Required Qualifications:
- Completion of Nurse Practitioner or Physician Assistant accredited program
- Current state RN/NP or PA license
- Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or National Commission on Certification of Physician Assistants (NCCPA)
- Access to reliable transportation
Preferred Qualifications:
- 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care
- Knowledge of long-term care, community resources, and cost-effective alternatives
- Knowledge of CPT, ICD-10, and HCPC codes
- Knowledge of clinical standards of care
- Awareness of NCQA requirements, CMS guidelines, Medicaid/Medicare contracts, and benefit systems
Connecticut Residents Only: The salary range for this role is $88,000 to $173,200 annually. Pay is based on several factors, including local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable. In addition to your salary, UnitedHealth Group offers benefits such as a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401k contribution (all benefits are subject to eligibility requirements).
