We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at IntelyCare in South Windsor, Connecticut. As an LPN, you will play a vital role in delivering high-quality patient care in post-acute settings.

Job Responsibilities:Your primary responsibilities will include:
- Providing compassionate nursing care to patients in post-acute settings
- Monitoring patients' physical and mental conditions
- Administering and verifying medications
- Maintaining a safe and clean facility environment
- Tracking residents' medical records and charts
- Collaborating with healthcare professionals to ensure seamless patient care
- Current Connecticut state license to practice as an LPN
- Completion of an approved LPN educational program
- H.S. diploma or GED (or equivalent)
- Minimum of 1 year of experience (LPN or related role)
