About the Role

We're seeking compassionate and dedicated individuals to provide exceptional care to our clients in the Queens area. As a Home Health Aide with CareConnect, you will play a vital role in ensuring the safety and well-being of our patients in the comfort of their own homes.

Key Responsibilities
  • Provide personalized care to patients, including bathing, dressing, and grooming, to promote their independence and dignity.
  • Assist with mobility and transfers, ensuring the safety and well-being of patients, and report any concerns to the appropriate medical personnel.
  • Administer medications as directed by healthcare professionals, maintaining accurate records and adhering to HIPAA regulations.
  • Monitor vital signs and provide emotional support and companionship to patients and their families, fostering a positive and supportive environment.
  • Maintain a clean and safe environment for patients, adhering to infection control protocols and ensuring a comfortable living space.
Requirements
  • Certified Personal Care Assistant (PCA) or Certified Home Health Aide (HHA) certification.
  • Understanding of HIPAA regulations and the importance of patient confidentiality.
  • Ability to cook simple meals and perform light housekeeping duties as needed, promoting a healthy and safe environment.
  • Satisfactory completion of New York State Health Department-approved HHA Training Program or equivalency.
  • Demonstrated adequate literacy and simple arithmetic skills, ensuring effective communication and record-keeping.
  • Must be at least 18 years old, with a valid ID and reliable transportation.
