Transform Lives as a Home Health Aide

A Home Health Aide plays a vital role in providing personalized care to patients in the comfort of their own homes. As a Home Health Aide, you will work under the direction of a Registered Nurse to deliver high-quality care and support to patients with various needs.

  • Assist patients with daily living activities, such as bathing, dressing, and grooming, to promote independence and dignity.
  • Provide medication assistance, including opening bottle caps, reading labels, and observing patients take their medications.
  • Take accurate temperature, pulse, and blood pressure measurements, and report any abnormal results to the supervisor.
  • Help patients maintain good personal hygiene by performing or supervising bathing, grooming, skin care, and other activities.
  • Assist with feeding patients, communicate basic nutrition principles, and observe and record food and fluid intake when necessary.
  • Support patients with toileting, including the use of bed pans/urinals, changing and positioning catheter bags, and bag change procedures on well-regulated ostomies.
  • Assist patients with mobility, exercises, positioning/turning, transfers, and ambulation, as per the Plan of Care and CenterWell Home Health policy.
  • Document interactions with patients, caregivers, doctors, and other staff members, ensuring continuity of care and accurate record-keeping.
  • Practice infection control principles to provide a clean, safe, and comfortable environment for patients.
  • Assist with household duties, including light laundry, bed changing, bed making, light meal preparation, light housekeeping, and shopping, as needed.
Requirements and Qualifications

To be successful as a Home Health Aide, you will need:

  • A high school diploma or equivalent.
  • Completion of a Certified Nursing Assistant or Certified Home Health Aide program within the last 24 months.
  • Meet applicable state certification requirements.
  • A valid driver's license, auto insurance, and reliable transportation.
  • Be in good standing on the HHA Registry (if applicable) and have completed HHA/CNA course to work for a Medicare-certified agency.
  • At least one year of experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, home health/hospice agency.
