Job Title: Home Health Physical Therapist

LHC Group is seeking a highly skilled and compassionate Home Health Physical Therapist to join our team.

About the Role:

The successful candidate will be responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment and evaluation, the Physical Therapist will work to determine a treatment plan, perform interventions aimed at improving and enhancing the patient's well-being, and evaluate the patient's progress.

Responsibilities:
  • Evaluate patient's functional status and physical therapy needs, assisting the physician in developing the therapy plan of care.
  • Observe and report patient's response to treatment and any changes in their condition to the supervising nurse and/or physician.
  • Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice.
  • Instruct and inform participating members of the healthcare team, the patient, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
Requirements:
  • Current Physical Therapy licensure in the state of Louisiana.
  • Current CPR certification.
  • Must have a current driver's license and vehicle insurance, and access to dependable transportation.
