Job SummaryThe LPN or RN will provide professional nursing care for residents of all ages in long-term care, under the supervision of a registered nurse, advanced practice provider, or physician. The successful candidate will demonstrate knowledge of nursing principles and provide care that is tailored to the individual needs of each resident.
Responsibilities- Provide direct patient care, including administering medications and treatments as ordered
- Conduct resident assessments and develop care plans to meet the unique needs of each individual
- Collaborate with the healthcare team to ensure seamless care and communication
- Document resident interactions and outcomes related to care, medication, behavior, activity, and dietary
- Participate in interdisciplinary care team meetings to discuss resident care and develop solutions to complex issues
To be considered for this role, you must have a current LPN or RN license with the State Board of Nursing where the practice of nursing is occurring. A minimum of 400 hours of experience as an LPN or RN is also required. Specific services/positions may have additional education, training, or experience requirements.
