Job Summary

We are seeking a skilled and compassionate LPN to join our team of healthcare professionals. As an LPN with Mercy Home Health, you will be responsible for providing high-quality nursing care to patients in their homes, under the direction and supervision of a registered nurse.

Responsibilities
  • Assess, plan, implement, and evaluate patient care to ensure optimal health outcomes
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams
  • Provide skilled nursing services, including wound care, medication management, and vital sign monitoring
  • Collaborate with interdisciplinary teams to coordinate patient care and services
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care transitions
Requirements
  • Current LPN licensure in the state of practice
  • One year of experience in a related field, preferably in home healthcare or hospice care
  • Ability to work independently and as part of an interdisciplinary team
  • Excellent communication and interpersonal skills
  • Flexibility and adaptability in a fast-paced healthcare environment
