Care Management Nurse

At Bel-Aire Quality Care - Newport, VT, we are seeking a skilled Registered Nurse to join our team as a Care Management RN. As a key member of our interdisciplinary team, you will play a vital role in delivering high-quality patient care and collaborating with healthcare professionals to achieve optimal patient outcomes.
Key Responsibilities:- Conduct thorough nursing assessments and work collaboratively with the nursing team to develop effective plans of care that meet the unique needs of each patient.
- Establish realistic patient goals and interventions to minimize rehospitalizations and evaluate outcomes, ensuring the highest level of care and quality of life for our patients.
- Delegate patient care responsibilities to staff and supervise them to ensure tasks and interventions are implemented effectively, promoting a culture of teamwork and excellence.
- Communicate patient information with assigned staff and between shifts, ensuring seamless care coordination and minimizing errors.
