Senior Geriatric Care Coordinator

We're currently seeking an experienced Senior Geriatric Care Coordinator to join our team in this full-time role.
Job Summary:
- Collaborate with RN case managers and nurse practitioners to provide high-quality patient care.
- Collect and document vital information about assigned patients, including medical history, medications, and test results.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless transitions of care.
- Assist with appointment scheduling, lab results, and diagnostic tests to support patient outcomes.
- Participate in Interdisciplinary Team (IDT) meetings to coordinate care and develop individualized plans.
- Conduct regular check-ins with patients and caregivers to address concerns and provide education.
- Utilize strong communication skills to build trust and rapport with patients, families, and healthcare teams.
- Stay up-to-date with industry developments and best practices through ongoing education and training.
- Meet company driving standards and maintain a valid state driver's license.
Requirements:
- High school diploma or equivalent
- Certified Nursing Assistant (CNA) program completion
- Minimum 2 years of clinical experience as a CNA in geriatric, adult, or family practice settings
- Current BLS certification
- Knowledge of community resources and ability to connect patients with necessary services
- Experience working with vulnerable adults or senior populations
- Ability to meet deadlines and prioritize tasks efficiently
- Demonstrated ability to work effectively in fast-paced environments and adapt to change
