Job Summary

Community Options, Inc. is a national non-profit organization providing residential group home, day habilitation, and supported employment services to individuals with disabilities in 12 states.

We are seeking a skilled Nurse Care Specialist in a community setting to deliver high-quality care to individuals with disabilities. The successful candidate will utilize the nursing process to assess, plan, implement, and evaluate care plans, while interacting with individuals, their families, and healthcare teams.

Key Responsibilities

  • Provide medical treatment and healthcare procedures as ordered by a physician and in accordance with professional practice standards and laws.
  • Ensure individuals attend medical appointments as outlined in their Individual Support Plan (ISP).
  • Communicate concerns and issues related to nursing services to supervisors.
  • Develop and distribute medical reports detailing general health, healthcare goals, and physician recommendations.
  • Collaborate with an interdisciplinary team to develop and update Individual Support Plans (ISP)/Person-Centered Plans (PCP) to address concerns, observations, and behavioral developments.
  • Train staff on the medical needs of assigned individuals.
  • Monitor health and medical conditions of individuals and refer for medical services as needed.
  • Manage medication administration logs to ensure accurate tracking.
  • Ensure health service supplies are available and replenished as required.
  • Maintain accurate and timely program documentation and billable records.
  • Build a support network for individuals by developing and maintaining relationships with community members, businesses, and families.
  • Participate in community, social, and recreational activities with individuals.
  • Participate in circles of support for personal futures planning.
  • Adhere to strict OSHA procedures regarding blood-borne pathogens.
  • Cooperate with the licensee and department staff in any inspection, inquiry, or investigation.
  • Perform additional tasks and responsibilities as assigned.

Requirements

  • Bachelor's Degree or equivalent experience in a nursing field.
  • Minimum of one year of experience providing nursing services to individuals with disabilities.
  • Valid nursing license.
  • Valid driver's license with a satisfactory driving record.
  • Complete all state and agency required training per state guidelines.
  • Ability to calculate figures related to medication dosages.
  • Understanding and commitment to community-based support for individuals with disabilities.
  • Ability to exercise good judgment and remain calm in crisis situations.
  • Ability to establish a comfortable and supportive relationship with individuals receiving support.
  • Proficient with Microsoft Office.
  • Excellent verbal and written communication skills.
  • Ability to effectively present facts and recommendations.
  • Ability to effectively communicate with program participants, third-party agencies, staff, and community members.
  • Excellent time management skills.

Working Conditions

  • Work in office, residential programs, day programs, and community environments.
  • May be exposed to changing weather conditions when working outside.
  • Hours and weekly schedules may change due to business needs and may include evening and weekend hours.
  • May be required to be on-call in cooperation with other management staff.
  • Occasional overtime may be required.
  • Frequent lifting, stretching, and other physical exertion may be required.
  • May be required to lift or move 50+ pounds.
  • May occasionally assist with wheelchair transfer of non-ambulatory individuals.
  • May be required to transport individuals using your own vehicle or company-provided vehicles.
  • May be exposed to various medical conditions, communicable diseases, blood, body fluids, or tissues.
