We are seeking a skilled and compassionate Home Health Registered Nurse to join our team at CenterWell Primary Care. As a key member of our care team, you will play a vital role in delivering high-quality patient care and support in the comfort of patients' homes.
Responsibilities- Provide comprehensive nursing care to patients with complex medical needs, including admission, case management, and follow-up visits.
- Collaborate with patients, families, and healthcare teams to develop and implement personalized care plans that address patients' physical, emotional, and social needs.
- Conduct thorough assessments, develop and implement care plans, and provide ongoing evaluation and revision of care plans as needed.
- Coordinate care with other healthcare professionals, including physical therapists, occupational therapists, and home health aides, to ensure seamless and comprehensive care.
- Communicate effectively with patients, families, and healthcare teams to ensure timely and accurate exchange of information.
- Document patient care and progress accurately and thoroughly, in accordance with regulatory requirements and CenterWell Primary Care policies.
- Diploma, Associate, or Bachelor's degree in Nursing.
- Minimum of one year of nursing experience, preferably in home health or a related field.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Strong organizational and communication skills, with ability to work effectively in a fast-paced environment.
