About the PositionAs a Nurse Practitioner or Physician Assistant with Matrix Medical Network, you will conduct comprehensive health assessments in the comfort of patients' homes, reviewing and observing their current health, medical history, medication adherence, social environment, and other risks.
This provides unparalleled insight into patients' overall health and well-being, allowing us to potentially close multiple care gaps with a single visit.
You will work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel to provide patient-centered care and improve health outcomes.

Responsibilities- Conduct comprehensive health assessments in patients' homes
- Review and observe patients' current health, medical history, medication adherence, social environment, and other risks
- Work collaboratively with healthcare teams to provide patient-centered care and improve health outcomes
- Document patient information and care plans according to Matrix guidelines and protocols
- Master's degree in Nursing or Physician Assistant studies
- Current RN and NP/PA licensure in the state of practice, including prescription authority
- Board certification in a Matrix-approved specialty
- 1 year of experience as a Nurse Practitioner or Physician Assistant
- Strong computer skills and familiarity with EMR software
- Excellent verbal and written communication skills
Ability to travel, valid state driver's license, and adequate automobile insurance coverage
Travel may be required, but we offer flexible scheduling to accommodate your needs.
