- Psychosocial Evaluation and Counseling: Provide psychosocial evaluations and counseling to patients and their families, addressing their unique needs and concerns.
- Record Keeping and Case Management: Maintain accurate and detailed records of patient interactions, interventions, and outcomes, ensuring seamless case management and coordination.
- Guidance and Support: Offer guidance and support to patients and their families, empowering them to make informed decisions and take an active role in their care.
- Interdisciplinary Collaboration: Work collaboratively with the interdisciplinary team, including physicians and management, to develop and implement comprehensive care plans.
- Master's Degree in Social Work: Possess a Master's Degree in Social Work from an accredited institution.
- Eligibility for NASW Membership: Eligible for membership in the National Association of Social Workers.
- Experience and Licensure: At least one year of experience as a Medical Social Worker in a medical or clinical setting, with a preference for experience in dialysis or other physically ill populations. Must be licensed in Alaska or willing to obtain licensure.
- Leadership and Organizational Skills: Demonstrate strong leadership and organizational skills, with the ability to prioritize tasks and manage multiple responsibilities.

- Physical Demands: Frequently stand, bend, stoop, walk, and sit, with the ability to lift up to 30 pounds.
- Travel and Accommodations: Valid driver's license and ability to travel to and from clinics as needed.
