Seven Hills Pediatric Center at Groton seeks a skilled Licensed Practical Nurse to provide high-quality care to children and young adults. As a key member of our team, you will be responsible for assessing and coordinating healthcare services, implementing medical interventions, and ensuring the safety and optimal health of our residents.

Responsibilities

• Conduct thorough assessments of residents' healthcare needs and develop individualized plans of care.
• Collaborate with the Program Manager, Registered Nurse, and Nurse Practitioner to ensure seamless care coordination.
• Provide transportation and assistance with medical appointments as needed.
• Maintain accurate and up-to-date health records, including medication and treatment orders.
• Communicate effectively with residential staff, school programs, and other stakeholders to ensure comprehensive care.
• Participate in staff meetings to present medical concerns and upcoming appointments.
• Complete mandatory in-service education and attend educational programs as recommended.

Qualifications

• Current Massachusetts LPN license.
• One year of experience working with medically fragile children.
• Valid driver's license required.
