Your Job Description

We are seeking a skilled Registered Nurse (RN) to join our team as a Caregiver and Health Advocate. As a key member of our team, you will be responsible for providing high-quality nursing services to our clients in the comfort of their own homes. You will work closely with our multidisciplinary team to develop and implement personalized care plans that address the physical, emotional, and social needs of our clients.

Key Responsibilities

  • Provide one-on-one care to clients in their homes
  • Develop and implement personalized care plans to meet client needs
  • Monitor and report any changes in client condition to the care team
  • Communicate effectively with clients, families, and caregivers to ensure seamless care transitions

Requirements

  • Graduate of an accredited school of professional nursing (BSN preferred)
  • Current, unrestricted license as an RN in the state(s) of practice
  • CPR certification and TB test
