About the Role

We are seeking a skilled Registered Nurse to join our team as a Hospice Care Specialist. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality patient care and support to individuals and families in need.

Key Responsibilities
  • Conduct comprehensive assessments of patients and families to determine their unique needs and develop personalized care plans.
  • Provide expert nursing care, including administering medications, treatments, and other interventions as prescribed by physicians.
  • Collaborate with the interdisciplinary team to coordinate patient care, including developing and implementing care plans, conducting regular assessments, and revising plans as needed.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination and address any concerns or questions.
  • Supervise and delegate tasks to hospice aides as needed, ensuring that patients receive the highest level of care and support.
  • Participate in quality assessment and performance improvement activities to ensure that our services meet the highest standards of quality and excellence.
Requirements
  • Graduate of an accredited school of nursing with a current Registered Nurse licensure in the state.
  • One year of hospice experience preferred, but not required.
  • Excellent observation, verbal and written communication skills, problem-solving skills, and basic math skills.
  • Nursing skills per competency checklist.
