Key Responsibilities:
  • Coordinate healthcare and wellness services for individuals, adhering to all relevant regulations.
  • Act as a liaison and advocate for clients' healthcare needs within medical and interdisciplinary teams.
  • Promote client wellbeing and ensure their inclusion in care decisions.
  • Train unlicensed personnel on medication administration and treatment protocols.
  • Implement and instruct staff on updated health policies and procedures.
  • Participate in medical appointments and relevant meetings as required.
  • Manage transitions of care between various healthcare settings.
  • Oversee medication management and pharmacy needs.
  • Maintain accurate health records and documentation.
  • Develop and revise individual health risk plans to ensure appropriate care.
  • Foster long-term relationships with individuals served.
  • Provide training to staff to meet the healthcare needs of clients.
Qualifications:
  • Current Indiana Nursing license
  • Valid driver's license
  • Preferred experience in community or long-term nursing
  • Experience working with individuals with IDD or mental health challenges
