Job Summary

The Home Health Speech Pathologist is responsible for evaluating, planning, and administering comprehensive speech therapy modalities to patients with speech, voice, language, cognitive-communicative, and oral-pharyngeal swallowing disorders in their place of residence.

Key Responsibilities

  • Evaluate patients to determine level of function and develop treatment plans to improve and/or restore speech and/or swallowing functions.
  • Provide initial and ongoing assessments to determine patient needs and progress.
  • Establish and implement in-home treatment plans in coordination with physicians and other care team members.
  • Plan, evaluate, and implement treatment for an assigned caseload of patients.
  • Provide quality, personalized, evidence-based care and proven interventions of Speech Language Pathology.
  • Educate, collaborate with, and conference with patients, families, and other healthcare providers.
  • Supervise Speech Therapy students and other home health staff as appropriate.
  • Complete required documentation to complete patient visits and episodes as required by federal, state, and payor guidelines.
  • Instruct patients and family/significant others in home programs.

Requirements

  • Master's degree or greater in Speech Language Pathology.
  • Successful completion of a program approved by the American Speech Hearing Association (ASHA).
  • Completion of Certificate of Clinical Competence (CCC).
  • One year experience in hospital or Home Care Setting, preferred.
  • Current ASHA license.
  • AHA CPR certification required.

Working Environment

This job operates in a variety of settings, including professional office environments, patients' homes, and cars.
