Job Summary

We are looking for a highly motivated and compassionate Traveling Nurse Practitioner to provide high-quality care to our patients in a home health setting. As a Home Health Care Coordinator, you will be responsible for delivering exceptional patient care, collaborating with our interdisciplinary team, and ensuring seamless transitions of care.

Responsibilities
  • Provide comprehensive hospice care to patients in their homes, focusing on pain management, symptom control, and emotional support.
  • Collaborate with our interdisciplinary team, including physicians, social workers, and home health aides, to develop and implement individualized care plans.
  • Conduct thorough assessments of patients' physical, emotional, and social needs, and document findings in a timely and accurate manner.
  • Develop and implement effective care plans that address patients' unique needs and goals, and make adjustments as necessary.
  • Provide education and support to patients and their families on disease management, medication adherence, and self-care techniques.
  • Participate in ongoing quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements
  • Current licensure as a Registered Nurse (RN) in the state of Texas.
  • Minimum of 2 years of experience in hospice care or a related field.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team, with a focus on delivering high-quality patient care.
  • Basic computer skills and proficiency in electronic medical records (EMRs).
