Key Responsibilities- Assess and Manage Patient Care: Conduct comprehensive health histories, perform physical examinations, and order diagnostic tests to determine client health status and formulate a problem list.
- Provide Coordinated Care: Develop and implement individualized care plans, collaborate with community health nurses and other medical providers to ensure continuity of care.
- Prescribe Treatment Modalities: Utilize your expertise to prescribe a range of treatment options to meet the health and illness needs of your patients.
- Collaborate with Interdisciplinary Teams: Work closely with healthcare professionals to ensure seamless care coordination and effective communication.
- Meet Productivity Standards: Maintain average monthly provider productivity standards, including but not limited to, number of visits.
- Adhere to Patient-Centered Medical Home Standards: Participate in all Patient-Centered Medical Home efforts and follow role assignments in alignment with these standards.
- Education and Experience: Graduation from a college or university accredited for Advanced Practice Nursing or Physician Assistant Training, with successful completion of an approved program of residency.
- Clinical Experience: At least 2 to 3 years of clinical experience, with a strong background in establishing and maintaining effective working relationships with patients, families, and healthcare teams.
- Knowledge and Skills: Possess knowledge of the principles and practice of medicine, as well as the structure and function of community health centers. Demonstrate excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
- Interpersonal Skills: Focus on solving conflict, maintaining confidentiality, and demonstrating empathy and respect for patients and colleagues.
- Quality Management: Emphasize quality improvement, accuracy, and thoroughness in all aspects of patient care.
- Professionalism: Demonstrate a commitment to professionalism, including respect for colleagues, patients, and the organization, as well as a strong work ethic and adaptability.
