We are actively seeking a dedicated Wound, Ostomy, and Continence Nurse (WOCN) for various employment options to provide exceptional care to patients across Montgomery County.

Key Responsibilities:

As a nurse with Jefferson Health at Home by BAYADA, you will utilize your clinical expertise and empathetic approach to assist patients in their preferred environment – their home. Your responsibilities will include:

  • Guiding clients through their healthcare journey – from hospital discharge to achieving a safe and dignified home life.
  • Providing individualized skilled nursing, interventions, and supportive care as per the tailored care plan, which includes medication administration, wound management, and comfort care.
  • Conducting wound care evaluations and offering professional oversight.
  • Performing patient assessments, educating patients and their families, and monitoring ongoing health conditions.

Qualifications:

  • Possession of a valid and active registered nurse (RN) license in the state of PA.
  • Wound, Ostomy, and Continence Nurse (WOCN) certification is mandatory.
  • A minimum of one year of nursing experience is required.
  • Home care experience is highly desirable, though not essential.
  • Willingness to travel to assigned cases.
  • A calm demeanor, effective listening abilities, strong assessment skills, and excellent communication capabilities.
