Job Title: Home Health Physical Therapist

Millennium Home Care, an affiliate of Millennium Healthcare, LLC, is seeking a dynamic and compassionate Physical Therapist to join our dedicated team. As a PRN Physical Therapist, you will have the opportunity to make a meaningful impact on the lives of individuals in need of home care services.


Responsibilities:
  • Conduct comprehensive evaluations of patients' physical conditions, including assessment of mobility, balance, strength, and pain levels.
  • Develop personalized treatment plans based on patients' specific needs and goals.
  • Implement evidence-based therapeutic interventions to improve patients' physical function and reduce pain.
  • Provide hands-on physical therapy to patients in their homes, ensuring their safety and comfort.
  • Monitor patients' progress and adjust treatment plans as necessary.
  • Collaborate with other healthcare professionals, such as physicians and nurses, to ensure coordinated and effective care.
  • Educate patients and their families on proper techniques for exercises and activities to promote independence and recovery.
  • Maintain accurate and up-to-date documentation of patient evaluations, progress, and treatment plans.
  • Stay current with advancements in physical therapy through continuing education and professional development opportunities.

Requirements:
  • Current state license as a Physical Therapist in Florida.
  • A Minimum of 1 year of experience in home care, or a related field.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong problem-solving and critical thinking abilities.
  • Valid driver's license and reliable transportation to travel to patients' homes.
