**Job Summary**
Cedars-Sinai Medical Center in Los Angeles, California is seeking a highly skilled Pharmacist to join our team in the Kidney/Pancreas Transplant Program. As a hybrid pharmacist, you will play a critical role in maintaining the quality and support of highly immunocompromised patients requiring high-level specialized services.
**Key Responsibilities**
- Maintain the quality and support of highly immunocompromised patients requiring high-level specialized services
- Manage nephrology consults, pre-transplant infusions for highly sensitized patients, post-transplant infusion patients, and cover in-patient services including managing non-formulary requests
- Enter therapy plans, conduct chart reviews, compose letters of medical necessity, and participate in peer-to-peer reviews for denied insurance claims
- Participate in in-patient rounding, including discharge teachings, medication reconciliation, ordering and coordinating last-minute medication changes prior to discharge
**Requirements**
- Pharm.D. or equivalent degree from an accredited school of pharmacy
- Completion of a PGY1 and/or PGY2 residency or equivalent practice experience
- Current pharmacist licensure in the State of California
- BLS certification as outlined in the Pharmacist Responsibilities in Cardiopulmonary Resuscitation (C.P.R.) and Code Blue Procedure: Pharmacy Policy and Procedure
**Preferred Qualifications**
- Completion of a PGY1 and/or PGY2 residency or equivalent practice experience
- Comprehensive knowledge of medical terminology as related to drug usage
- Comprehensive knowledge of common disease states and pathologies
- Ability to comprehend patient medical management plans
- Ability to assess and apply current pharmacological and biopharmaceutical principles for the selection and use of drug products in a clinical setting
