Job Summary:

We are looking for a compassionate and detail-oriented Registered Nurse to provide high-quality patient care in a home health setting. As a Registered Nurse Case Manager, you will be responsible for assessing patient conditions, developing individualized care plans, and coordinating care with the interdisciplinary team.

Responsibilities:
  • Assess patient conditions and develop individualized care plans
  • Coordinate care with the interdisciplinary team, including physicians, nurse aides, and LPNs
  • Administer medication as prescribed by the physician and help decrease re-hospitalizations by prioritizing visits for high-risk patients
  • Supervise and oversee the total care of patients provided by nurse aides and LPNs
  • Manage multi-disciplinary care as applicable while promoting continuity of care with appropriate admissions, transfers, and discharges
  • Provide education to patients and their families on the disease process using teach-back methods to ensure patient and family understanding
  • Complete all clinical documentation following agency protocol and Medicare/Federal guidelines
Requirements:
  • Registered Nurse with current license in the state of employment
  • Minimum one-year experience as an RN in an acute care setting. Home care experience preferred
  • Valid driver's license and auto insurance in your name as a driver
  • Capable of all physical demands
