Under the guidance of the Clinical Manager or designated representative, the Hospice Care Aide is responsible for performing essential patient care tasks that cater to the personal needs and comfort of individuals facing terminal illnesses across various environments.
As an integral member of the organization, designated hospice services, and the interdisciplinary team, hospice aides play a crucial role in ensuring the comfort and dignity of patients, providing care that aligns with the needs and objectives of both patients and their families, prioritizing patient-centric goals.
Key Responsibilities of the Hospice Care Aide include:
- Services are directed by the interdisciplinary group.
- Included in the individualized care plan.
- Permitted to perform tasks as per State regulations.
- Aligned with the training received by the Hospice Care Aide.
The Hospice Care Aide collaborates with the interdisciplinary group (IDG) to deliver high-quality patient care, with responsibilities that may include weekend and holiday shifts.
Primary Duties:
- Deliver hands-on personal care.
- Assist with simple procedures as an extension of therapy or nursing services.
- Support patients in ambulation or exercises.
- Help administer medications that the patient typically self-administers.
It is essential to report any changes in the patient's medical, nursing, rehabilitative, and social needs to a registered nurse, ensuring these changes are reflected in the care plan and quality assessment activities.
Additionally, accurate record-keeping in accordance with the organization's policies and procedures is required.
Provide education and training to family members or caregivers on proper techniques to meet the personal care needs of the patient.
Maintain patient confidentiality at all times.
Participate in team meetings and other agency meetings as directed.
Complete a minimum of twelve (12) hours of training/education specific to the position annually.
Other responsibilities may be assigned as needed.
Required Licenses/Certifications:
- Current licensure as a Certified Nursing Assistant (CNA) in the state of Florida.
- Current BLS Certification.
- Current AIDS 104 Certification.
- Valid State of Florida Driver's license with an acceptable driving record.
Reliable transportation is necessary.
Education/Experience:
Any combination of education and experience that equips the individual with the necessary skills and knowledge for effective performance is acceptable.
Typical qualifications may include:
- High school diploma or GED equivalent preferred.
- At least six (6) months of experience in home care, hospital, or long-term care settings.
- Experience in caring for terminally ill patients is preferred.
- Reliable transportation that allows for flexibility in work locations.
Provide one or more active contact numbers to enable management to reach the employee in case of unexpected changes in work location.
