The Float Nurse Practitioner will provide coverage approximately three weeks each month, serving Skilled Nursing Facilities across various locations in Ohio. All travel expenses are fully covered by the organization, and the specific schedule will be collaboratively determined.

This organization operates as a Medicare Advantage Institutional Special Needs Health Plan (ISNP) dedicated to long-term nursing home residents. The plan encompasses all medically necessary and preventive services under Medicare Parts A, B, and D, with a strong emphasis on enhancing members' physical, mental, and social well-being through tailored clinical care and therapies.

POSITION DESCRIPTION:

The Float Nurse Practitioner will deliver on-demand coverage to geriatric patients in Skilled Nursing Facilities. This role does not involve permanent assignment to a single facility; instead, the Float NP will rotate within the designated market to temporarily fill vacancies, cover for sick leave, and more. The specific travel parameters will be mutually agreed upon at the time of hiring.

The responsibilities of the Float Nurse Practitioner include:
  • Effectively managing the medical needs of enrolled SNF patients
  • Conducting thorough assessments
  • Creating and implementing individualized care plans
  • Participating in facility care team meetings
  • Communicating with family members and caregivers
  • Ensuring the care team is aligned with the member's goals and needs

As part of the clinical team, the Float Nurse Practitioner will receive support from the Clinical Manager and/or Director, along with the local Medical Director.

The organization utilizes an Electronic Health Record (EHR) system.

The Float Nurse Practitioner should anticipate traveling approximately three weeks per month, with all travel expenses covered by the employer.

REQUIREMENTS:
  • Ohio NP license
  • DEA certification
  • Minimum of 1 year of advanced practice experience
  • Minimum of 1 year of experience in geriatric care
