About the Role:

The Customer Service Representative is a vital member of our hospice care team, responsible for ensuring seamless patient access to our services. This role requires daily interactions with patients, families, and referral sources to provide detailed explanations of hospice services and facilitate the admissions process.

Key Responsibilities:

  • Acquire consents from patients and families for hospice services by clearly explaining the services offered, answering questions, and addressing any concerns.
  • Collaborate with patients, families, physicians, and referral sources to facilitate the hospice admission process.
  • Provide detailed explanations of hospice services, including eligibility, care options, and benefits, in various settings such as home, long-term care, and assisted living facilities.
  • Coordinate with referral sources, such as discharge planners and healthcare providers, to ensure timely and accurate communication regarding hospice services.
  • Maintain detailed records of interactions with patients, families, and referral sources, ensuring compliance with all legal and regulatory requirements.

Requirements:

  • High School Diploma. Some college required; Bachelor's degree in Marketing, Public Relations or healthcare-related field preferred.
  • Four to five (4-5) years' experience in marketing, sales, or healthcare relationship.
  • Valid Colorado driver's license and proof of automobile insurance in accordance with state statute.

We provide mission support services to better equip The Denver Hospice to operate as a distinct and independent organization while working together to share best practices and serve more Coloradans along the Front Range.
