Certified Wellness/Activity Director
- Must possess current certification as a Certified Activity Director.
- Experience as a Wellness/Enrichment Director in a skilled nursing facility is highly desirable.
- Design, organize, and execute a variety of individual and group activities tailored to the interests and assessments of residents.
- Prepare monthly calendars showcasing community activities.
- Thoroughly assess and complete assigned sections of the Minimum Data Set (MDS) in a timely and accurate manner.
- Utilize resident assessment protocols (RAPs) to evaluate the need for care planning regarding residents' activity program requirements.
- Document and incorporate residents' activity preferences, needs, and concerns into their care plans, ensuring regular reviews and updates at least quarterly.
- Record all interactions with residents and/or families in the assessment, care plan, and progress notes as mandated by federal and state regulations.
- Actively participate as a member of the interdisciplinary team.
- Facilitate and assist in organizing Residents' Council meetings.
