About the Role:

Gentiva Hospice is seeking a skilled Nurse Preceptor to join our team. As a Nurse Preceptor, you will play a vital role in providing high-quality care to patients and new hire clinicians during hospice visits.

This is a traveling role that supports our 8 Illinois offices in Central and Northern Illinois. You will be responsible for facilitating the learning experience to allow preceptees to perform competently in their required responsibilities, achieved through mentoring, coaching, and role modeling correct behaviors.

Key Responsibilities:

  • Performs patient home visits in accordance with physicians' orders.
  • Orients new nurses (preceptees) which includes hospice clinical policies and procedures, documentation, technology, compliance and regulatory requirements.

Requirements:

  • Current CPR certification.
  • Minimum 2 years' experience in a clinical setting.
  • Diploma, Associate's or Bachelor's degree in Nursing.
  • Unencumbered RN license in work state assigned.
