Outreach Advocate

Job Summary:

Oswego County Opportunities, Inc. is seeking a highly motivated and compassionate Outreach Advocate to join our team. As an Outreach Advocate, you will provide direct services to individuals of all ages, focusing on those who are homeless, at risk of homelessness, or experiencing mental illness, substance use disorder, or HIV/HCV. Your primary goal will be to provide comprehensive, non-judgmental, and consumer-centered services that empower individuals to achieve stability and independence.

Key Responsibilities:

• Provide prevention, initial contact, and intake services to individuals in need.
• Conduct assessments, develop individualized service plans, and coordinate services to meet the unique needs of each consumer.
• Offer ongoing crisis intervention, supportive counseling, advocacy, and solution-focused decision-making to high-impact target populations.
• Collaborate with other service providers to engage consumers and connect them to needed services.
• Develop and maintain positive community relationships and engage in public relation activities to promote outreach services and activities.
• Participate in training, staff meetings, conferences, and seminars to enhance your skills and knowledge.

Requirements:

• Associate's degree in a Behavioral or Social Science field, plus two years of experience working with children, youth, and/or families.
• Any equivalent combination of experience and training relevant to this position.
• Valid NYS Driver's License with a driving record within Agency policy and access to a reliable vehicle.
