At Atrius Health, we are committed to delivering high-quality, patient-centered care to every individual we serve. As a Clinical Pharmacist, you will play a vital role in optimizing patients' health outcomes and use of guideline-directed drug therapy through Collaborative Drug Therapy Management (CDTM), direct patient care, drug information consultation, academic detailing, cost-savings/quality prescribing initiative rollout, and clinician/staff education at practice(s) of responsibility.
**Responsibilities:**
* Provide clinical pharmacy support to clinicians and staff at our medical practices
* Optimize patients' health outcomes and use of guideline-directed drug therapy
* Participate in committees and workgroups to advance clinical pharmacy services
* Develop and implement organization-wide drug therapy management materials and tools
**Requirements:**
* Graduation from an accredited school of pharmacy
* Advanced degree (PharmD) and completion of a post-graduate year one (PGY-1) residency or equivalent ambulatory care clinical pharmacy experience
* Licensed as a Pharmacist to practice in Massachusetts
* Meet or be eligible to meet all criteria for CDTM per Massachusetts Board of Pharmacy regulations
