Job Summary

We are seeking a highly motivated and experienced Patient Relations Manager to join our team at Oak Street Health. As a Patient Relations Manager, you will play a critical role in educating patients about Medicare programs, resources, and affordable insurance coverage options available to them.

This is an exceptional opportunity for someone who is passionate about working with older adults and making a meaningful impact on their lives. You will be responsible for assisting patients with navigating Medicare and insurance issues, serving as an internal resource for insurance questions, and advocating for patients to ensure they receive the best possible care.

In this role, you will have the opportunity to work closely with our care team, outreach team, and patients to provide personalized support and guidance. You will also be responsible for managing the welcome visit and orientation process for new patients, educating patients on how to apply for public benefits, and supporting the clinic management team on operational activities.

Key Responsibilities:
  • Assist patients with navigating Medicare and insurance issues, including coverage, benefits, summaries, eligibility, and plan optimization.
  • Serve as an internal resource for insurance questions and concerns for providers and staff.
  • Advocate for patients to ensure they receive the best possible care and navigate the healthcare system effectively.
  • Manage the welcome visit and orientation process for new patients.
  • Educate patients on how to apply for public benefits, such as Public Aid and Extra Help for prescription drugs.
  • Support the clinic management team on operational activities, including scheduling and billing.
Requirements:
  • Bachelor's degree or equivalent experience.
  • At least 2 years of experience in patient relations, customer service, or a related field.
  • Strong communication and interpersonal skills.
  • Ability to navigate multiple computer programs, including Gmail, MS Word, Excel, and others.
  • US work authorization.
Preferred Qualifications:
  • Experience working with older adults and individuals with complex health needs.
  • Proficiency in non-English languages, such as Spanish, Polish, Russian, or other languages spoken by people in the communities we serve.
  • Customer relationship management (CRM) experience.
Salary Range:

$70,000 - $90,000 per year, depending on experience.
