Job Summary
The Registered Nurse Case Manager will be responsible for providing comprehensive care and services to a group of patients, visiting them in their homes, skilled nursing facilities, and assisted living facilities. This role requires a strong understanding of psych-social assessments, interventions, and community resources.
Key Responsibilities
- Provide high-quality patient care and services, including assessments, interventions, and coordination of care.
- Collaborate with the interdisciplinary team to ensure seamless care delivery and effective communication.
- Manage patient visits and documentation, prioritizing tasks and triaging multiple concerns.
- Develop and maintain strong relationships with patients, families, and healthcare professionals.
- Stay up-to-date with hospice rules and regulations, as well as electronic medical records systems.
- Travel across the service area to provide care, using reliable transportation and adhering to company policies.
Requirements
- Current Registered Nurse (RN) licensure in the state of Georgia.
- Previous hospice experience required, with a strong understanding of hospice care principles and practices.
- Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
- Strong time management and organizational skills, with the ability to prioritize tasks and manage multiple responsibilities.
