Nurse Case Manager

We are seeking a skilled Registered Nurse to join our Case Management team at Ascension St. Vincent's. As a Case Manager, you will play a critical role in coordinating patient care and ensuring seamless transitions from admission to discharge.
Key Responsibilities- Develop and implement comprehensive care plans that address patient needs and preferences.
- Collaborate with interdisciplinary teams to ensure effective implementation of care plans.
- Monitor patient progress and make adjustments to care plans as needed.
- Advocate for patients and families to ensure access to necessary resources and services.
- Licensed Associate Professional Registered Nurse (APRN) credentialed from the Alabama Board of Nursing.
- BLS Provider certification from the American Heart Association or American Red Cross.
- Diploma from an accredited school of nursing or required professional licensure at time of hire.
