Case Manager

We are seeking a dedicated and compassionate Family Services Coordinator to join our team at Homes for the Homeless. As a key member of our family services team, you will play a vital role in providing support and resources to families in need.
Key Responsibilities:
- Maintain and update digital and physical records of all families as required by governing agencies.
- Collaborate with clients to develop service plans based on an assessment of each family's needs.
- Assist families with referrals to outside agencies for employment, education, parenting, legal, medical, and mental health services.
- Maintain and update case files bi-weekly to document progress.
- Assist families with moving into permanent housing.
- Ensure that clients maintain an open public assistance case.
- Adhere to all city and state guidelines for maintaining documents.
- Conducts unit inspections.
Requirements
Bachelor's or Associate's Degree
Familiarity with case management is a plus.
Knowledge of Outlook, Word, and Excel.
Bilingual in Spanish or other language is a plus.
