We are seeking a skilled Physical Therapist to join our team at Sky View Rehabilitation and Health Care, a 192-bed Skilled Nursing Facility located in Croton-on-Hudson, NY. As a Physical Therapist, you will play a vital role in helping our residents achieve their highest level of physical, mental, and social well-being.
Responsibilities:
- Assist in planning, organizing, developing, and implementing Physical Therapy services in accordance with current applicable federal, state, and local standards, guidelines, and regulations.
- Plan, develop, organize, evaluate, and implement the programs, goals, objectives, policies, and procedures of the physical therapy program.
- Interpret and/or instruct in physical therapy policies and procedures to personnel, residents, family members, etc. as necessary.
- Participate in discharge planning, development, and implementation of resident care plans, resident assessments (MDS), etc. as necessary.
- Supervise Physical Therapy Assistants as outlined by the State Physical Therapy Practice Act.
- Maintain treatment records, resident files, and progress notes as required and maintain an adequate liaison with families and residents.
Requirements:
- Must possess a current license and registration to practice as a Physical Therapist, PT in the State of NY.
- Must possess current malpractice insurance, license in good standing.
- Dedicated to providing extraordinary compassionate care as a Physical Therapist, PT.
- Energetic, enthusiastic, committed, & passionate about quality care.
- Genuinely interested in helping ill, injured, disabled, elderly, emotionally upset, & sometimes challenging residents.
- Patience, a cheerful disposition, & willingness to assist any residents & co-workers.
- Excellent communication, customer service, & organization skills.
- Trusted to work independently & in a team environment.
- Able to work well under pressure, flexible, & will work harmoniously with all staff.
- Covid-19 vaccinations are encouraged but NOT MANDATED; we continue to offer COVID-19 vaccination based on eligibility guidance from CDC.
