Position SummaryAs a Family Medicine Medical Doctor at Heartland Community Health Center, you will play a crucial role in providing comprehensive primary care services to a diverse patient population, including individuals from infancy to the elderly. Our healthcare professionals adopt a holistic perspective towards patient care, emphasizing preventive measures, health education, and the management of both chronic and acute health issues.
At Heartland, we foster a collaborative and multidisciplinary environment that prioritizes patient-centered care, integrating various health services such as Pediatric, Psychiatric, Behavioral Health, and Dental Care.

- Conduct thorough medical evaluations, diagnose and manage both acute and chronic conditions, and provide continuous care for patients across all age groups.
- Encourage health maintenance and disease prevention through regular check-ups, screenings, immunizations, and patient education initiatives.
- Ensure accurate and timely documentation of patient records in accordance with medical standards.
- Develop tailored care plans for chronic conditions such as diabetes, hypertension, and mental health issues, ensuring effective monitoring and treatment.
- Educate patients and their families about health issues, treatment alternatives, and preventive strategies to foster long-term wellness.
- Collaborate with a range of healthcare professionals to deliver coordinated and comprehensive care.
- Engage in community outreach efforts, health fairs, and educational programs to raise health awareness.
- Participate in quality improvement initiatives aimed at enhancing patient care and outcomes.
- Advocate for patients' health needs, addressing social determinants of health and promoting health equity.
- Participate in on-call duties as required.
- Provide mentorship and leadership within the clinical team, guiding best practices and supporting quality improvement efforts.
- Adhere to HIPAA regulations to maintain patient confidentiality and safeguard personal information.
- Maintain professional certifications and licensure through ongoing education as mandated by certifying bodies.
- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) from an accredited institution.
- Completion of an accredited residency in Family Medicine.
- Board certification or eligibility in Family Medicine.
- Valid medical license to practice.
- Active Basic Life Support Certification.
- Previous experience in a community health setting is preferred.
- Ability to lift equipment weighing up to 25 pounds.
- Mobility throughout the facility to interact with patients and staff.
- Capability to remain stationary during patient interactions.
- Effective communication skills in English.
- Proficient in operating computers and standard office equipment.
