With a rich history dating back to 1875, the hospital has been affiliated with New York Medical College, making it one of the oldest partnerships between a hospital and a private medical school in the United States.

The hospital's mission is to deliver high-quality health services without exception, taking a person-centered approach that exemplifies the ICARE values: Integrity, Compassion, Accountability, Respect, and Excellence. Every employee embodies these values through empathic communication and partnerships with all individuals.

As an Ambulatory Care Specialist, you will work alongside a team of healthcare professionals to provide exceptional patient care. Your role will involve providing appropriate nursing interventions based on assessment, which may include instructing patients and/or family members/significant others regarding treatment, medication, diagnostic procedures, and the importance of follow-up visits. You will also observe and identify underlying and contributing factors/concerns, especially socioeconomic, cultural, physical, and language barriers.

In addition, you will make referrals to other departments and agencies as appropriate, such as Home Care, Visiting Nurse Service, Department of Health, Social Service, Dietary, etc. You will also participate in preventative health measures and case findings, maintaining health maintenance status of patients by reviewing necessary records.

Requirements for this position include a valid New York State license and current registration to practice as a Registered Professional Nurse in New York State. You must also hold or obtain a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA). For assignments in Post-Acute Care ONLY, individuals meeting specific criteria may be considered for appointment.
