Enhabit Home Health & Hospice is seeking a skilled RN Case Manager to join our team. As a key member of our interdisciplinary care team, you will be responsible for coordinating patient care, performing accurate OASIS collection, and ensuring the medication profile remains current. You will also consult with physicians and office staff as needed to provide comprehensive care to our patients.

Responsibilities

* Coordinate patient care and oversee the frequency of visits for each episode
* Perform accurate OASIS collection and ensure medication profiles are up-to-date
* Consult with physicians and office staff as needed to provide comprehensive care
* Attend weekly case conferences and monthly case manager meetings

Qualifications

* Currently licensed as a Registered Nurse (RN) in the state of employment
* A minimum of one year of clinical experience preferred
* Demonstrate knowledge and skill in current nursing practice
* Possess a valid state driver's license and automobile liability insurance
