Job Summary

We are seeking a skilled and compassionate RN Case Manager to join our team on a contract basis. As a Travel Healthcare Professional, you will be responsible for conducting intake assessments with patients and families, ensuring they feel equipped and supported, and developing individualized treatment plans that account for physical, financial, and other constraints.

Key Responsibilities
  • Conducting thorough intake assessments with patients and families to identify their needs and develop personalized treatment plans.
  • Ensuring that patients have access to necessary resources and support, including transportation to and from healthcare facilities.
  • Collaborating with healthcare teams to discuss patients' progress and develop effective care plans.
  • Verifying patients' insurance coverage and financial status to ensure they can access necessary treatment.
  • Developing innovative solutions or suggesting referrals for patients who cannot afford treatment.
Requirements
  • BLS and ACLS certification.
  • Sound knowledge of anatomy, physiology, pathology, and medical terminology.
  • Ability to stand for extended periods and possess strong technical skills.
  • Passionate about patient care and committed to delivering exceptional service.
  • Minimum of two years of current RN Case Manager experience.
  • Graduate from a fully accredited RN program and active unencumbered RN Massachusetts state license.
  • One year of Epic experience.
