About the Role:

We are seeking a skilled Hospice Nurse Case Manager to join our team at Jobot. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to patients and their families in the greater New Braunfels, TX metro area.

Responsibilities:

  • Respond to patients' and families' hospice needs, providing emotional support and timely interventions as needed.
  • Perform skilled nursing assessments of physical, emotional, psychosocial, and spiritual needs, documenting changes in patient condition and evaluating and reporting to the patient's physician and/or Medical Director.
  • Provide bedside nursing, perform nursing procedures, administer medications and treatments as ordered, and evaluate patient medication needs for palliation.
  • Cooperate with the patient's recertification process, evaluating and documenting the patient's medical condition and decline.
  • Assist the family and caregivers at the time of death, handling necessary arrangements such as pronouncing death, preparing the body, arranging for funeral home removal, notification of authorities, and medication and equipment disposal or removal.
  • Listen carefully to patients and families, timely requesting support from social work and chaplains as needed.
  • Document patient assessments, medications, treatments, and responses concisely, accurately, completely, and timely.
  • Participate in nurses', bereavement, and other meetings and activities, communicating with IDT members and facility staff members.
  • Coordinate and complete patient admissions, ensuring caregiver competence by instructing and verifying understanding of disease process, signs and symptoms of approaching death, equipment, supplies, and medication usage; safety measures, including body mechanics and universal precautions.
  • Evaluate and coordinate needed changes in level of care, including hospitalization, respite care, crisis care, and transfers.
  • Coordinate additional services such as referrals for contracted therapists and medical equipment/supply placement and removal.
  • Monitor caregiver stress and ability to care for the patient.
  • Report needed changes in the POC to the Interdisciplinary Team.

Requirements:

  • BSN prepared RN with experience in hospice care.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Excellent documentation and organizational skills.
  • Knowledge of hospice care principles and practices.
